Abstract Objective: The prognostic importance of Tumor-Infiltrating Lymphocytes (TILs) in the tumor microenvironment of various cancers is increasingly recognized. In the present study, we aimed to investigate the prognostic value of CD3+, CD4+, CD8+, and CD45RO + TILs and their relation to histopathological features in larynx squamous cell carcinoma. Methods: Formalin-Fixed and Paraffin-Embedded (FFPE) samples from 63 primary larynx squamous cell carcinoma patients were immunostained for CD3, CD4, CD8, and CD45RO expression. Positive cells in micrographs from Invasive Margin (IM) and Tumor Center (CT) of tissue specimens counted by ImageJ software and their correlation with disease outcome were analyzed. Results: The expression level of TILs subpopulations was associated with clinicopathological markers as well as Overall Survival (OS) and Disease-Free Survival (DFS). In multivariate analysis, high frequency of CD45RO + cells in IM were confirmed as an independent prognostic marker for DFS (p = 0.007, HR = 4.968) and OS (p = 0.007, HR = 4.957). Similar findings were observed in the multivariate analysis of the combined frequency of CD45RO+cells in IM and CT. Conclusion: TILs are associated with patients clinicopathological features. Also, our findings indicate that CD45RO + TILs are a valuable marker for risk prediction in larynx SCC and could predict patients’ outcomes.

Introdução: Acredita-se que o fator de crescimento vascular endotelial (FCEV) seja um importante fator angiogênico envolvido no crescimento, na progressão e na metástase tumoral. Objetivo: O presente estudo avalia a relacão entre a expressão tecidual e os níveis séricos e salivares do FCEV em carcinomas de células escamosas da cabeca e pescoco (CCECPs) e sua correlacão com aspectos clinicopatológicos. Método: Foram coletadas amostras de 30 pacientes com CCECP e de 24 voluntários saudáveis. Utilizamos análise imuno-histoquímica para a expressão tecidual e ELISA para determinação dos níveis séricos e salivares. Resultados: Não foi observada coloração para FCEV nos tecidos normais, enquanto que foi observada expressão de FCEV em seis pacientes (20%). O nível sérico médio de FCEV foi 83,7 ± 104,47 em pacientes e 50,04 ± 32,94 em controles. O nível salivar médio de FCEV foi de 174,41 ± 115,07 em pacientes e 149,58 ± 101,88 em controles. Não foi observada diferenca significativa pelo teste de Mann-Whitney entre controles e pacientes (respectivamente, p = 0,411, p = 0,944). Não foi observada relacão entre a expressão tecidual de FCEV e seus níveis séricos e salivares. Conclusão: A expressão elevada de FCEV foi observada em pacientes com CCECP, e isso sugere seu papel na patogênese de CCECP, mas não foi observada relacão entre a expressão tecidual e os níveis séricos e salivares desse marcador.

Introduction: Vascular endothelial growth factor is thought to be an important angiogenic factor involved in tumor growth, progression, and metastasis. Objective: The present study evaluated the relation between tissue expression, serum and salivary levels of vascular endothelial growth factor in head and neck squamous cell carcinomas, and their correlation with clinicopathologic features. Methods: Samples were collected from 30 patients with head and neck squamous cell carcinomas and 24 healthy volunteers. Immunohistochemical analysis was used for tissue expression and enzyme-linked immunosorbent assay was employed to measure serum and salivary levels. Results: No vascular endothelial growth factor staining was observed in normal tissues, whereas vascular endothelial growth factor expression was seen in 6 patients (20%). Mean serum level of VEGF was 83.7 ± 104.47 in patients and 50.04 ± 32.94 in controls. Mean salivary level of vascular endothelial growth factor was 174.41 ± 115.07 in patients and 149.58 ± 101.88 in controls. No significant difference was found by Mann-Whitney test between controls and patients (p = 0.411, p = 0.944, respectively). No correlation was found between vascular endothelial growth factor tissue expression and its serum and salivary level. Conclusion: Overexpression of vascular endothelial growth factor was found in head and neck squamous cell carcinoma patients, suggesting its role in the pathogenesis of head and neck squamous cell carcinoma, but no relation was found between tissue expression, serum levels, and salivary levels of this marker.

O carcinoma espinocelular (CEC) é a neoplasia mais comum do trato aerodigestivo superior. A interleucina-13 (IL-13) é uma citocina imunorreguladora com polimorfismos relatados para seu gene associados com a mesma doença, especialmente asma e alergia. O presente estudo investigou se os polimorfismos do gene da IL-13 (posições 1055C/T no gene promotor da 1L-13 e +2044G/T exon 4) podem distinguir os pacientes com CEC de cabeça e pescoço de controles saudáveis. MÉTODO: O estudo analisou pacientes com CEC de cabeça e pescoço (n = 137) pareados por idade e sexo com um grupo de controles saudáveis (n = 127). A genotipagem do polimorfismo do gene da IL-13 foi executada pelo método de polimorfismo no comprimento de fragmentos de restrição baseado em reação em cadeia da polimerase. RESULTADOS: Não foram identificadas diferenças estatisticamente significativas nas frequências dos genótipos e alelos entre pacientes e controles em ambas as posições (p = 0,16). Além disso, não foi observada associação entre os genótipos investigados e outros fatores prognósticos, como idade, sexo, localização do tumor primário, tamanho do tumor e tabagismo. CONCLUSÃO: O presente estudo sugere que não há associação entre os polimorfismos do gene da IL-13 (nas posições -1055C/T e +2044GI A) e suscetibilidade dos pacientes a CEC de cabeça e pescoço.

Squamous cell carcinoma (SCC) is the most common malignancy that involves the upper aerodigestive tract. Interleukin-13 (IL-13) is an immunoregulatory cytokine that has been reported to have some polymorphisms in it gene associated with same disease especially asthma and allergy. The present study aimed to investigate whether the polymorphisms of IL-13 gene (at positions of 1055C/T in the promoter of1L-13 gene and +2044G/T exon-4) differ in patients with head and neck SCC from healthy controls. METHODS: This study was investigated in patient with head and neck SCC (n = 137) and age- and sex-matched healthy controls (n = 127). Genotyping of IL-13 gene polymorphism was performed using polymerase chain reaction-based restriction fragment length polymorphism method. RESULTS: No statistically significant differences were found in the frequencies of genotypes and alleles between patients and control group at both sites (p = 0.16). In addition, no association was observed between investigated genotypes and other potential prognostic factors such as age, sex, primary tumor site, tumor size and smoking. CONCLUSION: This study suggests that there is no association between IL-13 gene polymorphisms (at position -1055C/T and +2044GI A) and susceptibility of the patients to SCC of head and neck.

O presente estudo tem por objetivo relatar as características, fatores prognósticos e desfecho de tratamento de 18 casos de melanoma da mucosa nasossinusal e conduzir uma revisão da literatura. MÉTODO: Entre 1995 e 2005, 18 pacientes consecutivos diagnosticados com melanoma da mucosa do trato nasossinusal e tratados em nossa instituição. Para encontrar as principais casuísticas de estudos relacionados, conduzimos uma revisão da literatura em PubMed e Scopus. No total, encontramos 35 casuísticas, incluindo 1.077 pacientes com melanoma da mucosa nasossinusal. RESULTADOS: Encontramos 16 homens e 2 mulheres. A idade de apresentação foi 51-80 anos (mediana de 58 anos). Todos os pacientes sofreram excisão cirúrgica seguida de radioterapia com ou sem quimioterapia. A mediana de sobrevida foi de 15 meses e a sobrevida geral de 5 anos foi de 23%. Sob análise univariada, estadiamento e remissão completa após tratamento inicial foram importantes fatores prognósticos. Entretanto, sob análise multivariada, somente estágio da doença teve significância estatística. CONCLUSÃO: Melanoma nasossinusal é um tumor raro e agressivo com alto fracasso locorregional e distante e pobre desfecho de tratamento. Entretanto, em uma revisão da literatura, encontramos significativa melhora em termos de sobrevida de 5 anos para casuísticas recentes comparadas com casuísticas previamente relatadas.

The present study aimed at reporting on the characteristics, prognostic factors and treatment outcomes of 18 cases of nasosinusal mucosa melanoma, and do a literature review on the subject. METHODS: between 1995 and 2005, 18 patients consecutively diagnosed with nasosinusal mucosa melanoma were managed in our institution. We reviewed the literature in PubMed and Scopus in order to find the main series from studies associated with this topic. We found a total of 35 series, involving 1,077 patients with nasosinusal mucosa melanoma. RESULTS: we found 16 men and 2 women, with age at presentation between 51 and 80 years (median of 58 years). All these patients were submitted to surgical excision followed by radiotherapy with or without chemotherapy. The survival median was 15 months, and the 5-year general survival had a percentage value of 23%. Considering our univariate analysis: tumor staging and complete remission after initial treatment were deemed relevant prognostic factors. Nonetheless, considering the multivariate analysis, only disease stage was statistically significant. CONCLUSION: nasosinusal melanoma is a rare and aggressive tumor, with high loco-regional and distant failure rates, and poor treatment outcomes. Notwithstanding, in a literature review we found significant improvements considering the 5-year survival for recent series when compared to previously reported ones.

O gene supressor tumoral p53 abriga um polimorfismo funcional que codifica ou arginina (Arg) ou prolina (Pro) no códon 72 da proteína p53. Este polimorfismo tem sido considerado associado com o desenvolvimento e prognóstico do carcinoma espinocelular de cabeça e pescoço (CECP). OBJETIVO: Foram avaliados genótipo e freqüências alélicas do códon 72 do p53 em pacientes iranianos com CECP. TIPO DE ESTUDO: Estudo de Caso. MATERIAIS E MÉTODOS: Um total de 132 pacientes com CECP e 123 controles saudáveis foram genotipados. A fonte de DNA foi composta de células mononucleares do sangue periférico. A amplificação do DNA foi realizada através da reação em cadeia da polimerase específica para alelos. RESULTADOS: A distribuição dos alelos e genótipos não foi significativamente diferente entre os pacientes e controles. Além disso, nenhuma associação estatisticamente significativa foi encontrada entre o polimorfismo do códon 72 do p53 e localização, sexo ou idade no momento do diagnóstico. No entanto, o genótipo Pro/Pro estava significativamente aumentado em pacientes no estágio IV (30,8%) quando comparado ao estágio I-III da doença (11,1%) (p=0,03), e um número significativamente maior de doentes com o alelo Pro teve um aumento no risco de desenvolver doença no estágio IV (OR=2,2, IC= 95% =1.2-4.2, p=0,01). CONCLUSÃO: Os dados revelaram que o polimorfismo do p53 não afeta o risco de CECP em iranianos; porém, pode afetar a progressão para um estágio superior tumor.

p53 tumoral suppressor gene harbors a functional polymorphism which codes either arginine (Arg) or proline (Pro) in the protein p53 of codon 72. Such polymorphism has been associated with the development or prognosis of head and neck squamous cell carcinoma (HNSCC). AIM: we assessed codon 72 p53 allelic frequencies and genotypes in HNSCC Iranian patients. STUDY DESIGN: Case Study. MATERIALS AND METHODS: a total of 132 HNSCC patients and 123 healthy controls were genotyped. DNA source was from mononuclear cells of the peripheral blood. DNA amplification was done by means of the allele-specific polymerase chain reaction. RESULTS: genotypes and allele distribution were not significantly different between patients and controls. Moreover, no statistically significant association was found between the 72 and p53 codon tumor location, gender or age at the time of diagnosis. However, the Pro/Pro genotype was significantly increase in stage IV patients (30.8%) when compared to stages I-III of the disease (11.1%) (p=0.03), and a significantly higher percentage of patients with the Pro allele had and a risk increase in stage IV disease (OR=2.2, 95% CI=1.2-4.2, p=0.01). CONCLUSION: data revealed that the p53 polymorphism do not impact the risk of HNSCC in Iranians, nonetheless, it can affect tumor progression to a higher tumor stage.

CONTEXTO: O tumor maligno da bainha neural do trigêmeo que acomete a cavidade nasal e os seios paranasais é extremamente raro. À microscopia óptica habitual sua identificação é difícil, sendo necessária a confirmação através de microscopia eletrônica e de técnicas imunoistoquímicas. RELATO DE CASO: A paciente era uma mulher de 41 anos com história de 10 meses de progressiva dor seguida de edema álgico em região facial à esquerda, e de sintomas de sangramento, secreção e obstrução nasal. Estudos com métodos de imagem sugeriam a presença de processo expansivo em cavidades nasais e paranasais à esquerda. À biopsia, os achados histopatológicos à microscopia óptica foram sugestivos de tumor de origem neural no trigêmeo. Estudos imunoistoquímico e por microscopia eletrônica confirmaram se tratar de neoplasia maligna da bainha neural do trigêmeo. Descrevemos as características clínicas, radiológicas e histológicas deste tumor, e revisamos a literatura.

CONTEXT: Malignant neural sheath tumors of the trigeminal nerve affecting the nasal cavity and the paranasal sinuses are extremely rare. With conventional optical microscopy, their identification is difficult, and it is necessary to confirm them by means of electron microscopy and immunohistochemical techniques. CASE REPORT: The patient was a 41-year-old woman with a ten-month progressive history of pain followed by painful edema in the left facial region, and with symptoms of bleeding, secretion and nasal obstruction. Studies with imaging methods suggested the presence of an expansive process in the left nasal and paranasal cavities. In the biopsy, the histopathological findings from optical microscopy were suggestive of a tumor of neural origin in the trigeminal nerve. Immunohistochemical and electron microscopy studies confirmed that it was a malignant tumor of the neural sheath of the trigeminal nerve. We describe the clinical, radiological, and histological features of this tumor and review the literature.
